As a CNA, Assist residents with Activities of Daily Living under the direction of a Registered Nurse. Assist in providing safe, clean, and efficient work environment. Provide quality nursing care to residents in a safe, clean and efficient environment which promotes their rights, dignity, freedom of choice, and their individuality.

CNA STANDARD REQUIREMENTS

  1. Supports the Mission, Values and Vision of Four Winds and the facility.
  2. Requires infrequent sitting, crawling, kneeling, climbing; occasional squatting/crouching; frequent balancing, reaching, twisting/turning, bending, stooping; and continuous standing and walking. Requires the use of hands with possible repetitive motion. Dietary Aide
PM Shift (2 pm -7 pm or after school) Holiday work is required (every other), as well as weekend work required (every other). Four Winds Manor in Verona is currently looking for a part-time Dietary Aide for our Food and Nutritional Services Department. Responsibilities include: Serving meals according to the dietary needs and requirements of the residents. Ability to communicate effectively with staff, residents, and the leadership team. Ability to carry out written and verbal directions. Resident interaction is encouraged. Flexible scheduling is available. Willing to train the right candidate!

Housekeeper

Perform housekeeping duties including but not limited to the following;

Follow the duties list as stated, Perform cleaning duties including dusting, mopping, vacuuming, washing windows, making beds, cleaning toilets, showers, sinks, and emptying trash, etc. Follow all isolation procedures set forth in the policies and procedures manual. Perform resident-requested duties pertaining to housekeeping. Deal with residents in a professional and courteous manner. Respond to changes in duties as assigned by the supervisor.

ESSENTIAL JOB FUNCTIONS:

  1. Performs according to established frequency standards, cleaning of the environment for patients, visitors, and staff (e.g. sweeping, mopping, dusting, trash removal, vacuuming, etc.)
  2. Utilizes principles of proper body mechanics and safety in providing the above service.
  3. Treats residents with dignity and respect. Maintains resident confidentiality by sharing information with appropriate facility personnel only.

Occupational Therapy Assistant

This a PRN/Casual Position

The Occupational Therapy Assistant (COTA) implements individualized therapy programs designed to restore, reinforce, and enhance performance under the supervision of an Occupational Therapist. The COTA assesses residents’ physical and mental abilities and analyzes the data to determine realistic rehabilitation goals. He/she will select activities necessary to help residents learn lifemanagement skills within limits of their mental and physical capabilities. In addition, the COTA instructs, educates, and trains residents and caregivers in those skills and functions essential for promoting independence. He/she directs activities to promote and maintain health.

Position Responsibilities:

  • Assists in the assessment, treatment, and implementation of patient care in accordance with the State Practice Act.
  • Assists resident under direction of an Occupational Therapist to maximize their potential in activities of daily living including areas of selfcare, meal time skills, and manipulation skills.
  • Develops appropriate treatment methods in collaboration with resident and caregivers, and implements occupational therapy treatment plan accordingly.
  • Occupational therapy services include but are not limited to:
    • Therapeutic exercise including aerobic capacity conditioning, balance coordination, body mechanics, postural stabilization, flexibility exercises, ADL and IADL retraining, strength, power, and endurance training.
    • Helps resident develop or regain physical or mental functioning or adjust to disabilities by implementing programs involving manual arts and crafts, practice in functional skills, homemaking skills, activities of daily living, and sensorimotor, educational, recreational, and social activities.
    • Use of electrotherapeutic, physical agent and modalities.
  • Meets resident care hours requirement for rehab gym, or as directed by Rehab Director.
  • Attends and contributes to resident care conferences and other related meetings.
  • Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing resident, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.
  • Documents resident care services by charting in resident and department records.
  • Maintains and promotes a positive attitude toward residents and their treatment programs.

Physical Therapist

Position Summary:

The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of residents. He/she implements individualized therapy programs designed to restore, reinforce, and enhance performance. In addition, the Physical Therapist instructs, educates, and trains residents and caregivers in those skills and functions essential for promoting independence. He/she directs activities to promote and maintain health.

Position Responsibilities:

  • Screens, examines, and evaluates residents, including history, systems review, and application of appropriate tests and measures, and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and designs a plan of care.
  • As necessary or applicable, supervision of Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
  • Meets resident care hours requirement for rehab gym, or as directed by Rehab Director.
  • Attends and contributes to resident care conferences and other related meetings.
  • Develops appropriate treatment goals and methods in collaboration with residents and caregivers, and implements the physical therapy treatment plan accordingly.
